- Your insurance card
- Physician referral forms, if required by insurance
- List of prescriptions and/or over-the-counter medication, including dose and frequency
- Previous urologic evaluations and/or other relevant medical records
- Appropriate x-rays and/or radiology reports
- Completed patient forms

Insurance and Payment Information
Urologic Consultants, P.C., is a provider for most major insurance plans, and provides insurance billing. If you have given us the necessary information, we will submit your charges to the insurance company. We will be reimbursed by the insurance company and we will bill you for the remaining amount that is your responsibility. If applicable, co-pay is due at the time of service. For your convenience we accept cash, checks, Visa, and MasterCard.
Charges not paid at the time of service are due within thirty days. We do understand that patients may experience financial problems occasionally. If you need to arrange an extended payment plan, please contact our billing office.
Our office does accept Medicare assignment, which means that the Medicare check will be sent to the physician. Billing to your supplemental insurance will be taken care of by this office if you provide us with the insurance information.
If you have questions regarding billing or which insurance plans we accept, please call us at the phone number below. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
